Rapid Parathyroid Hormone Assay
The Rapid Parathyroid Hormone (PTH) Assay, also known as intraoperative PTH monitoring or quick PTH assay, is a specialised test used during parathyroid surgery to assist surgeons in determining the success of parathyroid gland removal.
It is designed to provide real-time feedback on PTH levels in the blood during surgery. Its primary goal is to confirm the removal of hyperfunctioning parathyroid tissue (adenoma or hyperplasia) responsible for primary hyperparathyroidism.
This test is especially crucial in minimally invasive parathyroidectomy procedures, where the goal is to remove only the affected gland(s) while preserving normal parathyroid tissue. During the procedure, a baseline blood sample is drawn before the surgery begins. After the suspected hyperfunctioning parathyroid gland is removed, additional blood samples are taken at specific intervals, typically at 5 to 10 minutes post-removal. These samples are then rapidly analysed using a specialised PTH assay that provides results within 15 to 20 minutes. The assay measures intact PTH levels, which are typically elevated in patients with primary hyperparathyroidism.
